Are you a new patient?

Before your first appointment at the clinic, it is necessary to complete a form detailing your state of health. We also invite you to prepare a list of medications and/or natural and homeopathic products that you are taking.

How to get to the clinic?

Getting to the Centre Dentaire Fairmount is easy and convenient. We are located in the heart of the Mile-End district of Montreal, a neighborhood easily accessible by public transportation and by car.

Public transport:

• Bus: Bus lines 51 and 80 have stops very close to our clinic.

• Metro: The closest metro station is Laurier (orange line). From there you can take a bus or enjoy a short walk to our clinic.
