Before your first appointment at the clinic, it is necessary to complete a form detailing your state of health. We also invite you to prepare a list of medications and/or natural and homeopathic products that you are taking.

Getting to the Centre Dentaire Fairmount is easy and convenient. We are located in the heart of the Mile-End district of Montreal, a neighborhood easily accessible by public transportation and by car.
• Bus: Bus lines 51 and 80 have stops very close to our clinic.
• Metro: The closest metro station is Laurier (orange line). From there you can take a bus or enjoy a short walk to our clinic.
For your first appointment, please bring a valid ID, your health insurance card and any dental insurance information. If you have previous dental records, please bring those as well.
Yes, it is necessary to complete a medical form detailing your state of health before your visit. You can complete it at home to save time on the day of your appointment. If you cannot complete it at home, you can also do it at the clinic by arriving 10 minutes before your appointment time.
Yes, we recommend arriving approximately 10 minutes before your appointment time to complete any additional documentation and familiarize yourself with our clinic.
Our clinic is easily accessible by public transportation. Bus lines 51 and 80 have stops nearby, and the Laurier metro station (Orange Line) is a short walk or bus ride away.
